The Short Form Application was officially retired on August 31, 2025. Please use the Long Form Application for all new member submissions.
Two of President General Eisenman’s key objectives are to achieve a 10% increase in Supplemental Applications and a 10% increase in Coat of Arms Applications. The Short Form does not support these goals or the long-term strength of our membership, making this change both necessary and beneficial.
Rationale:
• Previously approved Short Forms may not be used as proof for a new Long Form Application.
• Coat of Arms Applications cannot be submitted from a Short Form.
• If your original application was a Short Form, any future Supplementals must be completed on a Long Form Application. This process is similar to starting a new application; however, documentation is not required for generations omitted from the original Short Form.
