Member/Supplemental Application Change

The Short Form Application is being retired effective immediately and has already been removed from the website.

Rationale:
• If your original application was a Short Form, any future supplementals must be completed on a long form application. This is like starting a new application, except the documentation does not have to be submitted for the generations omitted on the short form.
• Previously approved Short Forms may not be used as proof for a new Long Form or a Short Form application.
• You cannot submit Coat of Arms applications from a Short Form.

Two of President General Eisenman’s key objectives are a 10% increase in Supplementals and a 10% increase in Coats of Arms applications. The Short Form does not support these goals or the long-term strength of our membership, making this change is both necessary and beneficial.

I will review any Short Form applications that are already in my possession or have been postmarked by August 31, 2025.
